Hello
I want to have a form that can either be printed for those who want to snail mail it to me or it can be submitted at my website. This will be a simple basic information form.
Any advice you could give me?

Hi there,
In your cpanel (main page) > Fantastico, if you scroll down the menu on the left side you'll see phpFormGenerator. (I haven't tried it.) This is what it says:
Short description: A form generator featuring up to 100 form fields, all kind of input fields incl. file upload, customizable fields attributes, send submitted data to an email address or store them in a database, admin panel.
I was trying to figure out forms awhile back and didn't have much luck. At the time I didn't know we had the phpFormGenerator. (Maybe it's new?) I'm going to check it out when I have more time. I'm not sure if that's what you're looking for or not.
Maybe someone else will be by who has other ideas. (About forms, that is.:roll: )

There are several new things in Fantastico including phpformgenerator - I
did not know that, lol.
I tried the form generator and got it to create a form - just couldn't figure
out how to increase the text area sizes. I got the thank you page bit no
form response. This was not a fair trial - not enough time/energy right
now. (and not enough instructions, lol).

I don't recall ever seeing a form designed to give the visitor the choice of
email or snail mail. I guess someone could fill out the form, print the page
to snail mail, and clear the form information from the form without submitting.

The php form looks easy to use, But I have not tried it yet either. An alternative would be to create an editable pdf file , Acrobat Reader file. They would fill it out and e mail it to you or print it and snail mail it. This is not as clean, requires an extra step for user to send via e mail, but solves the print to snail mail problem. Or offer both - php to e mail, pdf to postal mail.
Google "create editable pdf" if you need a pdf creator program.

Hi,
I dl'ed a program called webformdesign. I was referred to try it from WebStudio forum.
So it is easy to use to make a layout but how do I get the submission button to submit info to my email?
Also I would like to have a password page for people who have submitted the form.
As far as a printable pdf file form. I dl'ed a pdf program that messed up a DLL file and it took me two days and hours of frustration to fix. So watch out for this one."PrimoPDF"

Forms need a "form handler" or "form processor" to tell it what to do, including where to send the form response on submit. It is not something that can be explained easily or quickly.
Form handlers can include a URL to a page after submitting. It is usually a Thank You page, but I guess you could set it to send them to a login page instead.
If you scroll about half way down this page, there are links to FAQs and Tutorials. http://webformdesigner.com/ Spend some time on the website for more ideas - they even have a form processor and a free form processing service.

From your first post, I thought a pdf would fullfill your needs, but needing it to go to a log in page is not a job for the pdf guy.
